Get a Sports Physical Exam
For students in grades 7-12, a physical examination must be completed prior to your student participating in a practice.
Physicals are good for the current school year as long as they took place on or after April 15th of the preceding school year.
There will be a section in the online registration for you to upload the physical.
A valid copy of the physical must be digitally uploaded here or your student will not be eligible to participate.
Note: Do not turn in paper copies of your student’s physical form to the office. You need to keep the form with you in order to upload an image during the registration process. Any forms turned in to the office will be returned to the student.
Registration Process
During the registration process you will be asked to provide general contact information, upload an image of your student’s physical form, review and agree to the activity handbook, provide medical history, and review concussion information.
You will need to create an account through Bound - please save your username and password for future registrations.
You may also complete registration on your mobile device by searching for “Bound Sports” on the app store. Registering on the mobile app makes it easy to take photos of your student’s physical exam.
If your student is in 7th and 8th grade and competing at the high school level, you will need to register them under the high school for that sport.

Billing
Once registration has closed, you will be billed through the FACTS Financial System where you can pay online or by check in the front office.
You may also choose to pay online via credit card, but there is a service fee for online payments.
Shiloh's activity fees help offset the cost of officials and travel expenses. Fees for the 2026-27 school are as follows:
- Sixth Grade - $90
- Middle School - $120
- High School - $150
