Things to Complete
Medication Authorization Forms must be completed and submitted to the school for the school to administer, or for a student to self-administer, medication during the school day. We ask that the form and medication be dropped off BEFORE the first day of school. (Back-to-School Night is a great time to do this!) These form can be found in the FACTS Family Portal, Shiloh App, or linked here.
- Prescription Medications Form
- Inhalers Form
- EpiPens Form
- Over-the-Counter Medication Form (i.e. Benadryl, Tylenol)
High school parents, please complete the Off-Campus & Student Transportation Authorization Form. This form is also required for students who will be driving themselves or riding as a passenger in another student's vehicle to attend POWER, Career Academy, or other off-campus activities when school transportation is not provided
A completed form must also be on file before students will be permitted to leave campus during FLEX and/or lunch.
The form can be submitted through the FACTS Family Portal or Shiloh App under 'Forms' or downloaded here and dropped off in the front office.
Please submit a Student Health Record Update if your student has an allergy or other medical condition that may impact them during the school day. Student Health Record updates can be submitted through the Shiloh App or FACTS Family Portal.
Before the start of the school year, please log in to the FACTS Family Portal or use the 'Financial' icon in the Shiloh App to review your student's lunch account balance and add funds if needed.
- Students entering 7th or 11th grade may have additional immunization requirements. Any required immunizations must be completed by October 1, 2026.
- Immunization records are shared electronically with schools by your healthcare provider, so there is no need to submit an updated immunization record.
- If you choose to exempt your student from immunization requirements, please submit the North Dakota Immunization Exemption Form to the school office. If you have already submitted an exemption form in previous years, you do not need to submit another one.

Policies & Procedures
As a PreK-12th grade school there is A LOT going on every day! Our goal is to keep parents informed and updated without flooding your inbox. To do so, there are several Communication Channels to be aware of and comfortable navigating. Here's a brief overview of our communication pathways.
Please take a few minutes to review our attendance policies in the Student/Family Handbook. If a student will be absent, please use the Student Absence Form in the Shiloh App or FACTS Family Portal to notify the school.
Please take some time to review the Student/Family Handbook each year as a refresher on our school policies and procedures. This handbook can be found linked here as well as in the Shiloh App and FACTS Family Portal under 'Resources.'

The Shiloh App is best for quick and easy access from your mobile device or tablet.
The FACTS Family Portal is best accessed from a desktop or laptop. It is accessible from a mobile device, but is not the most mobile friendly.
The FACTS Family Portal is what 'feeds' the Shiloh App - the information is the same.
